Statement

__.09.20__ of the year of JSC "______________________" has transferred to you the assignment of the right (claim) under the bank loan agreement №______________________ of __.05.20__, concluded between JSC "______________________" ( JSC "______________________") and ______________________. In accordance with clause 5, clause 2, Article 5 of the Law of the Republic of Kazakhstan "On Collection Activities" (hereinafter referred to as the Law), the interaction of the collection agency with the debtor and (or) his representative, and (or) a third party bound by obligations to the creditor under a bank loan agreement or a microcredit agreement must be provided by at the request of the debtor, information and copies of documents confirming the authority of the collection agency to carry out debt collection activities. According to clause 4, clause 1, Article 15 of the Law, the collection agency is obliged to submit information in writing about the balance of overdue and current amounts of the principal debt, remuneration, no more than once a month upon the request of the debtor and (or) his representative within ten working days from the date of receipt of the request. penalties (fines, penalties) for acquired rights (claims). According to clause 6, clause 5, Article 5 of the Law, when interacting with a debtor and (or) his representative, and (or) a third party, a collection agency is prohibited from misleading the person with whom the collection agency interacts about the amount, nature and grounds of the debt. Based on the above,

I ask you to:

Provide a copy of the contract of assignment of the right (claim) of debt on the DBZ №______________________ dated __.05.20__, concluded between JSC "______________________" and TOO "______________________ (______________________)";

Provide written information on the balance of the principal debt, remuneration, penalties (fines, penalties) ______________________ to the lender;
