Statement
on writing off accrued fines and penalties
________ years between gr. _______________________ ( hereinafter referred to as the Borrower) and JSC "________________" ( hereinafter referred to as the Bank) was concluded Bank loan agreement № _________, for which the Borrower was granted a loan in the amount of ______________ tenge, for a period of ___ months with an interest rate of ___% per annum. The borrower fulfilled its obligations in accordance with the Loan Agreement on time and in accordance with the established procedure. However, due to the delay in wages, and due to the lack of another source of income, the Borrower had to go into arrears. As a result, he received a penalty in the amount of ___________ tenge.

On June 26, 2019, the President of the Republic of Kazakhstan, Kasym–Zhomart Tokayev, signed Decree No. 34 (hereinafter referred to as the Decree). In accordance with paragraph 1, paragraph 2 of the Decree, the President of the Republic of Kazakhstan decided: The Government of the Republic of Kazakhstan and the National Bank of the Republic of Kazakhstan should take measures to write off accrued fines and penalties to citizens of the Republic of Kazakhstan who have unsecured consumer loans from banks and microfinance organizations as of July 1, 2019. In accordance with paragraph 1, paragraph 3 of the Decree, the National Bank of the Republic of Kazakhstan was instructed to take a set of regulatory measures to strengthen the responsibility of financial organizations, prohibiting the accrual of penalties, commissions and other payments related to the issuance and servicing of loans for all unsecured consumer loans of individuals with overdue debts of more than 90 days. Based on the above,

Write off accrued fines and penalties under the Loan Agreement № ___________ from ___________ of the year.
